It's all her fault for resisting; it also seems that her harness is not properly adjusted, I'm not sure though cause I've never gone skydiving (it's on the "bucket-list" though).

I would blame more on the instructor. The harness should have been double checked by hin right before you jump, cause they keep in loose while you walk to the plane and while you are waiting to be hooked up. Also you can see she was fighting him and it looks like he jumps over her to pull her out of the plane while she is sitting which I think caused her to become inverted in the harness. If someone gave me that much of a struggle I wouldnt feel safe with them and cancel the jump.

Not her fault. She's an old woman that doesn't know about skydiving, you can't expect her to know how the harness should be adjusted.
It was obviously loose before they went out the door. The instructor has the responsibility to make sure everything is right, not the "passenger".

They should expect that the "passenger" is likely to be terrified and unfamiliar with the equipment.

Would you hand an inexperienced rider a helmet and then go ride without making sure they had secured in right?

I was talking about here resisting so hard. Yes the harness was a little loose but she probably loosend it more when she fought so hard.

harness is just slack over her shoulders... I doubt it was tightened at the last second before the jump. That forward momentum of jumpmasters "climb over this hag, get the out of the plane" is probably what sent her out of the harness.

wonder what the percentage is of people who get forced out and say "glad jump master got me out, and i loved it" compared to ..."never doing that again, and all jumpers are whacko!"

I've never done it before but I feel that I would cooperate, trusting that they are professionals.
