fuggin frustrated please help

I'm trying to get the title transferred into my name from the bike I bought.

I have the title that the seller gave me when he payed off the bike, I have my bike insurance, my id, and a sales contract between him and I. I took it to the tax office and they said I still need a fuking Title Transfer Application. Isn't this what the purpose of signing the back of the title is for???

Anyhow, now I dont know the difference between the two forms I have, and which one I need. I have a VTR-346, and a 130-U.

Which one do I need and will either or work?

Thanks guys

Here is what you need. The only way this form is not needed is if you have a very detailed 'Bill of sale'

even with a very detailed bill of sail you need the vtr 13u almost all the time now.
