I think the ambulance backed over it. It looks like th rear of the bambulance is raised look at the rear wheel - fender gap..

Thats not a photochop, thats just a classic failure to control speed and/or following to closely. The two white scrape marks on the pavement were made by the front wheel when it pushed underneath the ambulance, also gives direction of travel. the puddle in the pic is probably fork oil or brake fluid. Pooling of fluids gives you a final resting point.....in this case underneath the ambulance. that grate folds up on all ambulances, so they can push the stretcher into the back. Those boxes on the back of ambulances are pretty sturdy and a low speed hit from a bike might not do any major damage at all. Remember momentum = mass x velocity.

Chances are the biker did ok. No bodily fluids or parts on the ground is always a good thing. Who knows... I've seen a seatbelt lacerate a guys liver in a very minor traffic accident, and he died 3 days later from internal bleeding.
